• Craven, Harrogate & Rural District PCT
• Hambleton and Richmondshire PCT
• Scarborough, Whitby and Ryedale PCT
• Selby and York PCT

NHS North Yorkshire and York is geographically the largest in the England, covering 3,200 square miles, including some of the most remote countryside anywhere in England.
Job details: We are looking to appoint an RGN with the Specialist Practitioner District Nursing qualification to work within our Virtual Ward to deliver care to patients living in the community. The post holder will have a wide range of clinical skills and be flexible. Evidence of relevant professional development is essential. You must hold the V100 Nurse Prescribing qualification and a teaching qualification. It will be expected that you have the up to date knowledge and the required clinical expertise to ensure that patients receive high quality, timely and evidenced based care in their own homes.
This post will provide the opportunity to take forward the expanding role of community nursing within the multi-disciplinary and primary care team to meet the long term conditions agenda and therefore an understanding and commitment to the modernisation of community nursing is essential. You will need to demonstrate knowledge of national and local issues and be committed to partnership working with other providers. Innovation and a flexible approach will be supported. Effective leadership, excellent time management and communication skills are essential. We are looking for someone with energy and commitment to deliver high quality nursing care and provide dynamic leadership to the team.

Travel is an integral part of this post, therefore access to own transport is essential.
