NHS North Yorkshire and York began on 1 October 2006.

It has been formed by the merger of the four original North Yorkshire Primary Care Trusts, which were:

• Craven, Harrogate & Rural District PCT
• Hambleton and Richmondshire PCT
• Scarborough, Whitby and Ryedale PCT
• Selby and York PCT

NHS North Yorkshire and York is geographically the largest in the England, covering 3,200 square miles, including some of the most remote countryside anywhere in England.
Job details: This is an exciting opportunity to join a multidisciplinary service, based in the community. The team is part of a comprehensive service which works closely with both social care organisations and the third sector.
The post holder will be responsible for a caseload of older people suffering from severe and enduring mental health problems.

You will also have a supervisory role with Health Care Assistants, and share student supervision.
You must hold a relevant professional qualification. You will be expected to assess, plan care and review under the Care Programme Approach,
As a qualified practitioner you must have at least two years post qualifying experience. You will be expected to have excellent communication skills at all levels. You will be able to work unsupervised and to have a sound knowledge and understanding of assessment, care planning and mental health legislation.

The post holder should be committed to the development of older peoples’ services and have a good understanding of the issues facing older people with mental health problems.
You must have a clean driving licence and access to a car at all times.
